Blues star Hazard has long been linked with a move to the Bernabeu and is believed to be seen as a possible replacement to star man Cristiano Ronaldo.
The Portugal international could be sold in order to fund the move for the winger, with Manchester United favourites to re-sign their former star.
Real are also keen on adding more creativity into their midfield with a possible move for Coutinho, who has also attracted interest from rivals Barcelona.
Brazil teammate and Barca star Neymar recently revealed that the Reds attacker would be a perfect fit for the current La Liga champions.
Both players could be keen on a change in scenery given their poor start to the campaign, with Chelsea sitting in 15th place and Liverpool in eighth.
