The controversial Italian only moved to the San Siro from Manchester City earlier this year, but is already believed that the club are frustrated by his on and off-field antics.
As a result, a January move is being mooted, which has alerted Chelsea boss Jose Mourinho.
‘The Special One’ worked with the now 23-year-old at Inter Milan, and is thought to be a big fan of the unorthodox forward.
Milan value ‘Mad Mario’ at around £25m, which is seen as a bargain rate by the Blues chief for a player with such potential.
Balotelli enjoyed a successful spell with City prior to returning to his homeland, helping the Sky Blues to lift the Premier League trophy in 2011.
But, his performances on the pitch were spoiled by some moments of odd behaviour off of it, frustrating the club’s staff.
Mourinho is thought to be looking for a top class centre-forward to lead his attack, and could turn to Balotelli this winter.
Could Mario Balotelli be the man to fire Chelsea to league glory?
